The Fundació Sant Joan de Déu (FSJD) will lead the European guideline for the transition of young cancer patients from paediatric to adult hospitals within the YARN project
The Fundació Sant Joan de Déu (FSJD) is taking part in the European Youth Cancer Network (YARN) project, an initiative that expands on the work previously developed in EU-CAYAS-NET and OACCUs to strengthen the European network of young people affected by cancer. The project promotes peer support, the use of digital tools, and access to reliable information, especially for those facing relapses, metastatic cancer, or treatment-related side effects.
Within YARN, the main role of the IRSJD will be to lead the development of a European guideline for the transition of young patients from paediatric hospitals to adult hospitals. This guideline will address one of the most critical moments in the care pathway, ensuring a structured, safe transition aligned with psychosocial, educational, and clinical quality standards. The development of this resource will be carried out in coordination with professionals, young patients, and families from different European countries, ensuring it reflects real needs and diverse experiences.
The project also emphasises youth empowerment through the Youth Cancer Council, a body that will systematically integrate young people with lived cancer experience into decision-making and project evaluation. In this way, their perspectives will have a direct impact on outcomes and on long-term public policy recommendations.
Additionally, YARN will study the role of social connections in young people's emotional well-being and will conduct an impact study to analyse collaboration among organisations and synergies with other European projects. Through focus groups, peer visits, training programmes, webinars, and workshops, the project will support the implementation of psychosocial care standards, provide educational and employment support, and generate resources for long-term follow-up and transition care.
The project will also promote Equity, Diversity and Inclusion (EDI) using tools such as the Inclusivity Checklist and a certification system for healthcare institutions and patient communities. It will also include physical activity programmes, workshops, and a European bootcamp to encourage healthy lifestyles.
YARN's communication campaigns aim to reach more than 50 million people across Europe, and the ACE Academy will train over 100 young patient advocates. At the same time, the project's multilingual digital platform will be expanded to offer reliable information and strengthen connections with other EU-funded initiatives.
With this leadership role within YARN, the FSJD reaffirms its commitment to improving the well-being, continuity of care and quality of life of young people living with cancer in Europe.
